I just drop stance and then shirk. Im not fighting someones' ego.
Quite honestly, I do not see much difference between Ot and Mt anymore.
In savage/extreme if i die as MT i want things to stay in order (other tank might get confused if i dont provoke) so i provoke as fast as i can and wait for a shirk. If i'm OT then i stay OT.

this will probably cause issues for your healers and and possibly lead to you dying again, better to delay a little while so you can get topped, if you wanna keep the order of who holds the boss.
On normal mode, the bosses that do just automatically target whoever is a tank automatically. As an example, Shinryu normal mode always autos both tanks while on ex mode he autos whoever is the two highest on his threat list.
It's less about MT or OT and more about which tank is going to pull. In certain savage mode fights due to the timing on various cooldowns and how you want to space them out will decide which tank will pull.

For consistency of CD's there mostly is a reason why a certain tank is tanking a boss. But overall for AA's sake allow the better-geared tank to main tank.
Now regarding people who want to fight you for agro, I usually play along. It can be fun ^_^!

When I'm off-tanking a boss, I usually just keep stance off and provoke whenever the boss is casting something to maintain second on the enmity list but without the boss ever actually switching to me.
That way if the MT does die, they can just provoke the boss back once they've been topped off and regain the enmity lead immediately.
