You may want to try some of the following if you haven't already:
-Try a a clean reinstall of your graphics card drivers to reduce the potential of conflicting configurations.
-Disable any overlays (Geforce Experience, Discord overlay, etc.) that may be present while playing the game. Also make sure to close any other programs that are running alongside the game while playing.
-Perform a clean boot. Simply open the task manager and select the 'Start up' tab. Disable everything here and restart the PC. This will make sure that no background processes are interfering with the game.
When you reinstalled the game, did you remove the locally-saved data folder (located My Documents > My Games > Final Fantasy XIV)? If not, try removing it, but be sure to make a backup of this just in case it doesn't resolve the issue.
