It has occured to me that during my time doing the quest the children had to be kept inside of the sick ward, and the guard outside was wearing a mask....

Has it finally come to Eorzea?...

Pretty sure that ward was part of a different quest in the past. I can't remember what for though. I only remember knowing of the ward long before this year's Little Ladies event was active.

It was! I remember that.

It was also part of 1.0 which is why it was brought back as a little reference to FFXIV's past both in this year's Little Ladies Day and a previous Christmas event. As for the attendant wearing a face mask, plenty of NPCs wear masks in the game, and as this one is an ALC working in a children's hospital, it makes sense he's wearing facial protection (also remember Eorzean medical 'science' is still stuck somewhere in the medieval era long before modern germ theory).

Hells, AST's AF hat has a mask on it (well, veil, but that's basically the same thing).
