4 Expansions later and tab targeting is still bad.

The only time I find it really problematic is in large scale PvP or 24 mans when I'm trying to target someone outside my party to heal. It's really difficult to click target characters that are moving for some reason. The other way to target them requires a click on the alliance UI but that's even worse because you have to scan the entire thing to figure out where that one person in that mess is if there are multiple of the same job in the same party. Usually they're dead by that time in PvP. For mobs I can just use the enemy list or camera manipulation.

Clicking all of your abilities AND your targets with the mouse alone is the most optimal way to play.

I'm using numbers; SHIFT + numbers; CTRL + numbers; 4 mouse buttons; the letters "Q, E, R, T, F, G, V, C, X"; SHIFT+those letters as well as CTRL+those letters and I still don't have enough spare keys to assign all my emotes. Assigning abilities to keys is out of the question, all abilities need to be clicked if I am to play (RP) optimally.

Personally, I can't even fathom even attempting to do that, let alone actually playing like that.

But, since this is all a matter of preference anyway, the keyboard is a no-go either IMO.

12 button mouse + modifiers. And it's difficult to run out of hotbars because you can combine modifiers. Got shift, alt, and ctrl already mapped? Use a hotbar for ctrl+shift.

Tab targeting in FFXIV - oh there is an extra mob attacking you directly in front of you ... so you must want to attack that enemy 300 yalms away from you ... i got you fam! you're welcome!

Whether its any good can be debated

Whether it can be fixed is not. There would be no way to make the game not tab targetting without rebuilding from the ground up. And if they did that, they'd be justified in making it a brand new full priced game. Hell, they might as well make it ff16

It's still the worst tab targeting system I have ever used. Out of all the problems in this game that is one that should have been fixed ages ago. Hate on me all you want, call me names and how stupid and spoiled I am coming from WoW but, tab targeting in this game is garbage and needs to be fixed.

There aren't a lot of good alternatives though; clicking is too clunky.

As there have been for 8 years, or, as you like to call it, 4 expansions...

Character configuration > Control Settings > Target > Target Type 2 : Cone

Yet y'all can't be bothered to search in the settings for 8 years and counting. And this post will be lost, might as well add it to my signature...
