Also, I can remember two regular Eulmoreans who have the larger model Dulia-Chai and a random npc who wanders the town who you can't interact with. Nothing about them would suggest they over eat but their size, and people can be naturally heavy without overeating.

People can naturally be a bit on the larger side without overeating. No one ends up properly obese unless they're eating more than they need though.

Plus, given we only see fat people in Eulmore, they're clearly meant to represent the city state giving itself over to sloth and overindulgence.
