A very long stretch and probably not what everyone has in mind, but different variations for facial skin and taint would be nice. An example would be Dragon Age Inquisition, the option to give your character flushed or simply unclear skin and the likes.

So far, everyone is either a perfect facial skin model or a mess of wrinkles.
