All 3. In fact, this is one of the reasons why FF XIV is my favorite MMO. You can use both controller and mouse/keyboard, and use keybinds for both. Runescape and WoW you could only use a mouse/keyboard, which made for some not-as-good functionality (at least as far as I can remember. It's been ~5 years since I've played them).

I play with Controller and Keybinds, on KB there are only my Def-CD's or Skills i don't use in Combat, like Limit Break.
For me it's easier to see when my CD 's are back up to use, so i can focus on Battle and Movement with my Controller
Growing up, we couldn't afford getting consoles so PC games with keyboard+mouse are what I'm most natural at.
Nowadays, I do use controller on some games. I've tried it with XIV for a while. But the lack of muscle memory (slow at pressing the trigger+buttons) makes me uncomfortable to use it during combat. I just use controller when gathering.
My keybinds usually includes the number pads and Home/Insert/Delete/etc plus the 2 side buttons on my mouse.
I wish I can get an MMO mouse but I have small asian hands. lol Those I've found (that I can try hand held it in person) seems too big.

Keybinds and mapped to MMO mouse. Used to use F-keys mapped to LH macro keys which I could tap with my pinky finger from WASD but as they've reduced skill count I've eliminated that. Tried controller hoping I could floof on the couch with laptop but I found the camera waaaaaaaaay too slow on there, like...glacially slow. I liked the targeting and didn't mind the multiple modifiers for the cross bar (MMO mouse plus hotbar only needs one) but it's not for me.

I use mouse and keyboard - controllers have always been extremely difficult for me to move with, I'd never be able to dodge mechanics with one. Plus there's barely enough room on my keyboard for my rotation, I'm honestly not sure how it would work on a controller without excessive modifiers? Also I like being able to type and chat while I play, so I think I would quickly find it unbearable to have to put down a controller each time I wanted to do that.
I've bound what will fit, but I still click a few things since there's just too much on the bars for some jobs - stuff like Raise, LB, and consumables I click to conserve space.
But yea, me trying to accomplish anything while using a controller would be like an infomercial failure compilation. I can't use them, but I love KBM anyway so it doesn't matter.
I'm very much a keyboard and mouse person. My MMO experience started with Tibia, then RuneScape, then World of Warcraft, then Runes of Magic (WoW clone), then Guild Wars 2, then finally FFXIV. So keyboard and mouse felt natural to me. I use a combination of these 2 devices for my gameplay. Use my keyboard for typing mostly.

Genuinely not trying to be rude with that either. I use one myself and while it takes some getting used to, it's nice being able to talk on the Xbox without having to put down the controller every time.
White Mage ~ Scholar ~ PaladinBoi if you got kicked for the same thing in over 20 duties I strongly suggest you think hard on whatever the hell it is you're doing
As I'm sure you are well aware, it takes more than one person to be able to kick a player from a duty, so in all those instances there were at least two people agreeing they'd be better off without you tanking.
