i have the sameproblem on Asus GTX 690
used drivers: 301.33 and 301.34
both gived me the directx error. after setting of the AM and DOF i dint crash for 3hours in uldah (had to log off)
when AM off and DOF on i crashed after 1hour in uldah.
for people who are using 301.34 you cant shutdown normaly your pc. it's a known bug and they are working on it. with 301.33 you can normaly shutdown your computer.

POSSIBLE FIX:
DISABLE HARDWARE ACCELERATION FOR MICROSOFT SILVERLIGHT
This worked for me, apparently all the ingame menus are silverlight based. Added bonus: the menus seem snappier.
I was crashing every 5 minutes in uldah, every 10-20 minutes in other places. Now im not crashing at all, i even turned general draw quality up to 9 (dof+ao off, will test later with them on) and game not crashing. I just sat in the middle of a bunch of ppl in uldah and crafted stuff for 15 minutes and my card didnt even hit 60% usage, clocks seems much steadier. Im pretty sure silverlight and the game engine fight for control of the graphics device and eventually silverlight takes over and makes the game crash.
This would also explain why Nvidia cannot reproduce the issue. Either they are using older silverlight, have hw accel off, or dont have silverlight installed at all.

was having a frustrating evening today .. so sorry to the full party just now (Marsha Mallow, Mussa Gray, Ayu Marimo, Madison Sublime, Thalim Ahongle and etc .. which we wait for a full party, shouting around for quite some times)
eventually when we have the full party we travel to the grinding or level spot .. and poof~ screen turn black and FFxiv was stop functioning itself due to UNKNOWN DIRECT X problem ..
Oh my god i was so sorry to the party as im seeking solution from SE and internet as well, im getting a newest version of direct x from microsoft also .. hope this kind of problem doesnt repeat, its alright when you are solo-ing and doing leves by your own and shutting down issues doesnt bring trouble to others but annoying to self, however when this kind of issues happen when we are in a party or doing something together, it might cause lots of misunderstanding and problem ..
PS* i did not Tab+ Alt , and im playing in full screen
guess im having the same issues as yours, where my bf is playing BF3 without any problems in the computer but when im playing FFXIV its keep on crashing

Yeah i tried the same way. And it worked only if i lower the setting worser than my 580. It's not the point lowering the graphisms when you bought a new card. Even more the famous 680...
Gpu-Z told me the crash happening this way : Stable state (50-60% gpu power used) then it burst in 2-3sec to 99%+
Something going wrong like if there was some kind of data bottling if you know what i mean. That does not seem to have to do with overclocking, power, or else. Mine was not overclocked at all. The data managnement only is compromise, so it's really a coding problem. May be hypothetic but i am **** sur myself...

mmmm so jelly of your 680 gtx. Cant wait till i have enough gill to get one.
Awaiting response from Nvidia tech support (I am too upgrading from 580 to 680 but my 680 has not arrived yet, I will report back here tomorrow if I get these crashes)
