Adrenaline requires risk. Permadeath doesn't actually necessitate risk, however, unless one has an unstable connection or goes into very particular fights while teamed with people who can PK you or while the whole party is undergeared. It just means playing at a less intense pace as not to be killed by something stupid. The risk mitigation would rarely be a part of combat so much as a limitation on which content is worth doing.
There would be risk, though. I stated in my post that for the game mode damage would be increased to make combat more of a threat across the board. Like in POTD/HOH it should be AoE's that are avoidable that deal a significant amount of damage, though auto-attack damage should be increased by a reasonable amount too.
