Apparently everyone i talked to that had issues, used Comcast "US" as a internet provider.... So theres something with comcast and Square having issues.

Lag went away for an hour and came back even harder
Yeah can confirm. Im in the strange situation of having both comcast and a frontier connection in my house right now. It was unplayable anywhere on comcast so out of curiosity I swapped back to frontier. Viola. No lag. Further proof comcast needs to burn down.

I am *not* using Comcast and can't stay connected tonight. It's not just this game either, although it is not every game (I'm not having problems with Guild Wars 2, for example, but can't stay connected with a friend on TeamSpeak).
Problem started around 6 PM eastern time. Everything was normal smoothness prior to that; the change was rather abrupt.
I don't think it's necessarily ISP specific, but more dependent on the route your traffic is taking. As others have noted everything seems to start falling apart on the gin.ntt.net hops.

Saw on Reddit a few people mention Comcast. . .
.
.
.
I've also Comcast. . .
.....yay..
People saying this lasted a year, that's terrifying lol. I assume everyone is going to pass responsibility off on someone else but theoretically if I were to call about this.. who'd I call?
Could it be caused by massive amounts of bots logging into ffxiv and out that's causing this bag lag. I use desert wind internet service and when check my ping when lag starts inside duties jumps from 9 to over 100+.
No one that will be able to fix it. We aren't a customer of NTT so NTT will not listen to us. Square Enix won't fix it because all they'll tell us are copy paste answers off of what I call "The Script". The only solution I found was to play super late at night or use a VPN.
It happens like this almost all day (over a 10 minute graph), and only once it leaves my ISP network. I use ATT.

Could be failed hardware, most likely scenario in fact. I work for a telecom, and one of our recent failures took days to repair, due to the hardware being EoL and them having to scavenge a part off ebay basically. It happens sometimes, as ISPs aren't really encouraged to keep their crap up to date, the expense is more than the downtime and complaints. which is why NA has such terrible internet infrastructure. No one that will be able to fix it. Can anyone else confirm that this is happening on the European Data Centers as well? I've been having the same issues since yesterday, but if no-one else has been having it, then it just issues my end.
Massive spikes. Having to wait 30 secs or longer for MSQ NPCs to reappear after ending a cutscene or instanced quest. or being killed by dungeon mechanics as my character was lagging behind despite me moving correctly to avoid AOE or boss mechanics
