Considering plenty of people post pictures of their modded characters on the official forums, and they have not been banned, I'd say Squeenix probably doesn't care too much about it.
It's technically against the ToS, but they have no way actually detecting it unless you admit it in game or on the forums.
As long as you don't enter official contests or upload videos or stream with mods you should be ok?
FFXI was perfectly fine with mods save for contests and allowed Windower screenshots for contests so... (for mods it was allowed since mods are client-side only, and don't give the player benefits over other players)
it's not allowed, but REALLY it's only not allowed cuz squeenix doesn't want to field every customer service call when some newby modder breaks their game. "we told you not to..."
most of these mods are literally undetectable and they REALLY don't care.
Even if you did mod your client to change your character's appearance, it wouldn't help much for roleplay because only you could see the changes you made.
Modding is a grey area, officially its not allowed (as botters or exploiters might try using this a defence etc) BUT in practice noone in SE will bother you over this if you dont say anything as it's purely cosmetic and has absolutely no effect on the game mechanics wise. Generally the mods I've seen are all just basically costume mods or HQ textures that improve the look of the game and generally none are malicious but there's no support if things go wrong and you have to redownload the entire client again due to corrupted files.
Even WoW doesnt ban over this their more infamous ban wave a while back was reversed quickly because it bacame apparent the detection method used to detect cheaters was also flagging modders who were using a similar method to add custom textures to the game though they warned people not to continue using them since it risked being banned by mistake.
Generally if you wish to mod its at your own risk though.
Almost noone ever got banned purely for using windower unless they were yelling all over chat because even though it was 3rd party software it's developers didn't allow addons that could be used for cheating it was all purely performance stuff and cosmetic stuff too, wow like functionality like addons for showing TP/distance/running the game at 60FPS instead of its native 30 (that last one makes the game look so much cleaner too!). Truthfully it's stuff like that which help to prolong older games.

If you're just using cosmetic mods you're probably fine, though I tend to not use them because I don't like having that disconnect between what I see in game and what other people see of my character in game when we RP.
its something that is done on your end and only viewable by you so wile it is against the tos but technically speaking if you dont tell anyone no one will know your doing it but yes its is against the tos
Whether or not it's against the ToS, unless someone reports you, I doubt anything will happen. I'm pretty sure SE has better things to do than aggressively pursue texture modders. I imagine that the defacto policy is a don't-ask-don't-tell one. As long as the modders aren't gaining an unfair advantage against other players, I don't see why it would be worth SE's time to even ban texture modders. As long as you're not impacting other players' experiences, there's no incentive to enforce the ToS.
