Not sure if this is a bug used a conseal on floor 28 and monster still saw me and attack me when was invisible. Says its suppose to mask you from all enemies I thought. One those roaming head wheels saw me and attacked me.

Pretty sure some monsters will always see you regardless. I recall that also going on with Ninjas using their stealth.
Used again on floor 29 and this time same type of enemy didn't see me and attack. I think they need to fix it where works on every monster in there.
Did you use it near an enemy? Did you use anything else besides sprint while stealthed?
I've been to HoH floor 100 four times in total and never had an issue unless I was close to an enemy when it was actually popped.

only done it in a party so don't know if different agro types exist there but the game has also sound agro for blind enemies and other types of agro as well, you just need to learn which enemy has which type.
It's tough to say without seeing the circumstances. I think in Eureka some mobs that aggro by sound will aggro by walking through them, I'm not sure if touch is a type of aggro separate from sight/sound, but maybe conceal is sneak/invis only and you bumped into it and aggroed?
Even if you are walking slow to prevent triggering by sound aggro, any mob in the game will still trigger if you go directly infront of them. Usually these mobs have distinct features that let you decide where it's front is. You have to imagine a little cone infront of them therefore..
Concealment however hides you completely. Using skills, opening a mimic, using other pomander are actions that stop the buff, besides its duration.
When I remember now, I think on that level are mobs that target you randomly with aoe visible or not. Like the fat guy that rings a bell or the elemental that hits you from far away with a little aoe under you. If you get hit you get out of Concealment. Later you have these allagan turrets, mammoths roar, apes chest bumps or worm sandstorms and I bet even more.

The player went directly infront of it and it aggrod in that video. Serverlags/ticks makes it look like the player is only behind the mob shortly before, but it passed its center for a brief moment. Also when the player moved earlier close infront of him, the mob turned. When a mob turns he won't aggro, for some reason, until it turned fully. I'm pretty sure it would aggro if you would stand at his ring at the arrow that shows its front.That's not true, some mobs are sound only.
