The current 24man can be extreme/savage already depending on the rest of the players. /s The closest we had to your idea was The Baldesion Arsenal, and while the mechanics weren't hard to call it extreme or savage, they were very punishing. Combined with the fact that BA punished you really hard for dying. Going harder than that would be a pain, considering that it's complicated enough to keep an 8-man static together nowdays. 24-man would be a pain in the rear, imo.
That's the problem, hard enough to both gather and coordinate 8 people to do challenging content, I can't imagine 24.

I'm not sure how feasible it would be to coordinate three whole static groups in a concerted effort, and if the current Alliance raids like Orbonne are any indication then pugging an EX/Savage variant would likely be impossible.
Some people can barely handle The Thunder God as it is.

At some point you wipe so much in a 24 man that you don't even leave solely because you already wasted an hour and 30 and want to feel like that time at least meant something.
Extreme Alliance Raid could be fun, done right.
If it was me I'd look at how to make it flexible raid size. For example 12 - 24 man. Make it pre-made only. Then we might have something that could finally encourage FC raiding. Maybe even add a small incentive bonus or a 5% buff for a full FC group, nothing to force you into an FC but enough to give an incentive.
There's a lot of things I dislike about WoW, but guild raid nights were incredible fun and I really miss that.
