Risk in MMORPGs have been gradually removed because it doesn't go well with the genre. Remember in the classic days when it was a given in MMORPGs that dying not even makes you lose EXP, items, and money, but also delevels you, plus you had to run back naked to your corpse to pick up your gear, or visit the bank first to equip your backup gear so that you even stand a chance at reaching your corpse to recover your primary gear. The risk you're looking for isn't something new. It is a very old concept that has proven to not work in this segment of the market.
