However, I am also bamboozled at the idea of someone only ever running using M1+M2 held together.
That's an interesting choice for movement.
It's a choice that gives you analog control over your direction. I do it too on the rare occasions that I play on PC, where I only have a keyboard and mouse set up. Mouse movement isn't as good as a controller, but it's a thousand times better than trying to trying to navigate a twisting path using just the keyboard's straight line movements. (But I really don't see how --or why-- anyone would attempt non-trivial content without a controller.)
Yeah... mouse movement and turning is better... I don't think anyone's going to seriously argue that. But the keyboard still exists and has movement keys. The difference (in combat anyway, the only time precise movement really matters) is what holding W does to the availability of your left hand and all the fingers on it. Holding W means one of your fingers isn't available to conveniently press a hotbar key without weird contortions your doctor would advise against.
But there are times for everything. There are times it's ok and even preferable to do that instead of holding both mouse buttons (for instance when you need to click something else with the mouse... like the loot window).
Personally though, I use an Orbweaver and use the thumbstick for that type of movement.
