There's a few exceptions, where your quest's next objective doesn't have a location. Like if the objective for one quest is to complete another quest, you have to look at that second one to find where to go. (The OP has a few examples of that. If there's a quest telling you to do a leve, it's the leve itself that you want in your quest tracker, rather than the initiating quest.)
There are also a few quests that do not provide any quest markers or similar help at all. They tend to be some type of puzzle quests, and you are always clearly told when you pick them up that the game won't give you any help.
