You dont have to play it if you cant, try out playing on PS4 controller instead of keyboard.

Would help out the one handed players.

I'm good with 1-6 with WASD only for movement, granted I am using a mouse with 18 buttons. QEZXC are also convenient binds. Shift, Control, and Alt keys effectively double triple and quadruple your keys. Assuming ~ + 1-5 + QEZXC, 44 binds are available without modification to WASD.How exactly do you expect anyone to be able to reach 6 through = without using a second hand on the keyboard? OP send me a screenshot of your skill bar.
I use mouse for movement , QWEASDZXC for keybinds.
The problem is that keybinding WSAD to any modifiers will stop movement while that modifier is being held. Even for a split second, that could be the moment of escaping a mechanic or dying.

If you dont want to sacrifice wasd, I'd either recommend besides control, using shift for another set of keybinds or binding some stuff in the mouse buttons if it allows. (No need for those fancy mmo mouses, one with 2 extra buttons should suffice looking at your UI)
That's the thing though. My hands hurt like hell if I use ctrl or shift too much, so I keybound those to the two side mouse buttons I am comfortable using. I don't have any extra modifiers I can use aside from ALT...If you dont want to sacrifice wasd, I'd either recommend besides control, using shift for another set of keybinds or binding some stuff in the mouse buttons if it allows. Physical limitations suck and I commiserate with the difficulty you are encountering, however I feel it is less of an issue with the game itself and more just general difficulty of finding a setup that works for you.
Since your issue seems to stem from potential limited mobility with your hands that results in pain, perhaps using the traditional mouse+keyboard setup is not the way to go.
Here are some potential solutions that may help you out.
Using a controller.
This requires much less stretching and moving of your fingers and many players even prefer it while playing on their PCs because they find it more comfortable over long play sessions. The game actually has very good controller support for an MMO and with some good key-mapping it can work incredibly well. If you can get your hands on a good controller that has programmable extra buttons, you could even set those extra buttons as the shift/ctrl/etc modifiers.
Getting a gamepad
Gamepads like the Razer Orbweaver or this option from Hori could help you fit more keys and abilities all at your fingertips.
I myself use the orbweaver and couldn't imagine playing the game without it. I have my basic movement mapped to the thumbstick and then about 3 full hotbars of abilities bound to the keys, using the left-most column as the modifier keys that I can easily press with my pinky and then the other four columns as the actions themselves.
I also use a Razer Naga where I bind my most commonly used offensive actions, but you expressed you have difficulty with the sidekeys on a mouse like that which leads me to ...
Try another gaming mouse
In addition to using a gamepad, you could potentially try using a different gaming mouse to see if it's extra keys are easier for you. Since you have difficulty with a gaming mouse that has a 12-key pad on the side, you could perhaps try one of the ones designed for MOBAs instead. They have half as many keys and the keys are much larger and arranged in an easier to press layout. While it is fewer keys, it could still be more keys that you could actually use.
Lastly, and this may not even apply to you and your specific situation, but never underestimate the benefit of good writs and hand support for extended gaming/computer use sessions. I find that a lot of the instances where people begin to develop or experience wrist and hand pain while using a computer, it is because they are putting excessive strain on their wrists which in turn results in strain and pain in the ligaments of their fingers which can eventually result in permanent damage. I always recommend that anyone that uses a computer for long periods of time, whether for work or gaming, get themselves a good wrist cushion that is comfortable during extended use. I personally prefer the ones with the small beads inside them as I find that they cushion and cradle my wrists better, making for better longterm comfort. If the pain gets really bad, sometimes wearing a wrist support style glove can help quite a bit.
Anyways, those are the things that I can think of off the top of my head that may help you.
