The Fae took the title from the former King of Voeburt when they moved in. They saw that the ruler of the mortals was called a King, so they took the name for their ruler as well, regardless of who or what their ruler happens to be at any given time.
Titania is referred to as both king and queen, but mostly by king, I only remember seeing queen in the quest where you're given the dress. Given the exclusive usage of "they" (at least in the English client) by the pixies (& co) it appears they don't really care about using gendered terms. To them the leader of the fae people is the king.

Pixies are pretty much agendered from what I can tell and only use the terms King and Queen as it pleases them due to mortal influence. Plus they're basically the reborn spirits of children. Children don't care about proper word usage so much as what sounds better, and I imagine /King/ does when considered in conjunction with what Espiritduo stated.
All of the Fae are genderless, as far as I could tell. Pixies and Nu Mou both use "singular they" pronouns.
I'm also pretty sure that King in certain cultures is considered gender neutral, though even for those its usually males who get the title anyways. So it isn't all that out of place.
There is also the fact that the title King in some cultures is genderless and refer to the leader of people, regardless of gender. We've had a few women in our history who were called king because of this. The Pixies, who don't seem to have gender as we understand it, would logically follow that type of custom.
The term King is more gender neutral. This is the same thing that happened in Berserk's story their Fae King is actually a "Queen" Flowerstorm "King"
Really ? That's so weird, in the french localisation, Titania is called 'Queen'.
That being said, there aren't any gender neutral terms in french to name someone (beside 'it' ? sounds rude lol). All of the pixies are refered to as females in the translation.
Is it the same thing in the german localisation ? I'm curious now![]()
It's "König" in german which is clearly the male word for a ruler
Male: "König" - "King"
Female: "Königin" - "Queen"
I first thought this to be a fault by the ger. localisation team, which bugged me, cause "Every child knows that Titania is the Queen of Faires and Oberon is the King!"
But hearing it's the same in other languages and then with the explanation it just bugged me even more. The Pixies are genderless and named the title after the King of Voeburt? This is just so unreasonable. Why did the devs come up with this, just to change it from Queen to King? The Pixies also could've named it after the Queen of Voelburt.
