I had this happen just once today. I logged in after an unusual super long loading screen (like 2 minutes maybe), went to talk to my retainers, clicked one and they didn't show up. After sitting there locked in that interaction for about 10 seconds it said something like "unable to view scene" (I didn't get a screenshot and it was quite a few hours ago) and then it exited the entire summoning bell menu. When I went to interact with the summoning bell again I got the error and crashed.
