Brd quests Requiem for the Fallen

Thread Title:
During the brd quest Requiem for the fallen, when some players in the party leave the area after obvious failing, players dead in instance do not get removed from the instance, furthermore when you disconect from the game, log on fails. Has stopped players logging in for up to an hour so far that I have experienced.

* Fight in instance
* Players Die
* Some players run out of instance
* players dead disconnect rather than needing to return all the way back to home point
* unable to log back in with (40000) 6 error
