BLM Quest "Always Count on Black"

During this quest as well as other job quests that require instanced fights, if someone exits the fight, those who died in the fight remain and are stuck. Sometimes re-logging works but today with three re-logs, my party members had to hit Return and ride back to location.

Three of our Japanese party members also encountered problems where we all entered the fight but then they were instantly kicked out. We would all have to run back out of the fight and retry.

* Fight in instance
* Players Die
* Some players run out of instance
* players dead disconnect rather than needing to return all the way back to home point
* Re-logging doesn't always fix the problem therefore makes players have to hit Return
