100% Dungeon Completion

I can't remember if I dreamt this, but I swear you could 100% search a dungeon and get a little popup or something to indicate you did it. This is a thing, right?

Also, is there any point/way to keep track of which are 100%'d, and do I have to search the whole dungeon in one go, or can this 100% be achieved over multiple runs.

Bonus question, Paladin's Sword Oath, is the damage added to my auto attack or is it a viewable separate instance of damage?

#1 You can get an achievement for uncovering the whole map of a dungeon. You can do this over multiple runs.

#2 It's an added second Autoattack. You can proof this yourself by turning on Sword Oath and attacking a training dummy and you will see 2 damage numbers popping up for your Autoattack instead of the usual single one.
