There was only 1 title I remembered that had a spoiler and that was changed pretty quickly.

Once is enough to prove that titles can contain spoilers. If you don't want to be spoiled, it's not worth the risk having them on. If they did it once, they can do it again. It doesn't matter that checks were put in place because they may not realise it's a spoiler until seeing it from a different perspective.
