ive got the same problem, it was caused by Corsair iCUE software, that used up 30% of cpu usage.
please check it via task manager.

I check for memory leaks and if anything is killing my CPU. the game itself is around 30-40% and Icuq is running 0.1 to 0.6 % of my CPU workload.
I wish it was that simple as an iCUE issue.

By the Twelve, I turned off Fullscreen Optimization, like...years ago? (playin since launch) for similar issues. I did notice awhile back that I was gettin the odd hitch here n there and my GPU temps and utilization was higher than normal. I run Vsynced at 60 just to keep temps down and my monitors, though very good for image quality (pro quality IPS, 10 bit colour etc) are only 60Hz. However when I see these issues I will unlock and see what my max is, was barely breakin 80 FPS on a GTX1080 max settings 2560x1440. Didn't fuss much over it since I'm real busy irl and synced it never really bothered me, thought mebbe it was due to an update or summat and thus unavoidable (old system, decent GPU). Dropped HBAO to Strong from +Quality and moved on. Saw this thread, checked settings fer ffxiv_dx11.exe and sure enough...ENABLED. Poor form MS....poor form. Anyhoo, fixed that n FPS is up to 125ish in the same zone....so, thanks? I was actually gonna rebuild my base system soon, going to rag it out a bit longer now xD
Don't touch me there
