my account has suspicious activity and have to change my password....???

.... i change it and can be login again, but what happen with it, i kind of worry,i play on PS4 and i still play normal up until few hours ago, then i have to go out for a hour, when i came back home and turn on my PS4, it had HDD failure so i have to factory reset a whole system, reinstall PS4 software and FF14 all over again, when i log into the game i receive that message
