Don't know if this helps you or not but im running a EVGA 460 GTX SE @ 60-70 degrees under 90%+ load, setting are very similar to yours, but I do have DoF off. Also have a Coolmaster HAF you mentioned, you wanted.

Now my CPU on the other hand is different. Under 60%+ load my cpu temp reaches high 80's to sometimes low 90's. Freaks me out. CPU is i5 2500k. Redid my thermal paste on stock heatsink, thinking about aftermarket heat sink now...

Does anyone know how much difference Windowed to Full screen mode helps?

Oh man I understand that situation. I had something similar which my CPU, though it was an AMD Phenom II instead. The stock cooler and the paste it came with were both garbage, running hot and noisy whenever I launched FFXIV. I bought some good paste (in a nail polish bottle), paste remover, and a nice aftermarket sink. Now it's quiet and never goes above 45C. It was running 60-70C before.

Fullscreen definately helps a bit (maybe 5-8%). Limiting frame rate to 30 helps alot but fast movements won't be as smooth anymore.
Getting a 3rd party heatsink is definately a must for highend CPUs. The stock fans are always cheap, small and loud. Don't forget to check if the heatsink you want fits into your case 'cause some are really big. With HAF this shouldn't be an issue though.
The optimal solution would obviously be watercooling, but not everyone has 200-300 bucks loose :P
