I've also gotten an out of range error when trying to raise someone immediately after I've been revived when I haven't yet moved. If I move, then the problem goes away.

Can you give me an example of what kind of enemies you're having issues with? You snagged my curiousity. Since I don't normally cast from on top of something, I went out and thundered some coblyns and beetles from on top of them.. and other than the very uncomfortable feeling of having a spiky spider between my legs, everything worked as normal for me. :<
Not often since mobs often twitch and turn around. But it can pretty much happen with any mob if you get lucky. Easier to reproduce the problem with dead people and raise.
It happens all the time to me as gladiator that I get the out orf range error when standing inside something like a crab or right on top of a cactuar.

Thank you for posting your concerns about being unable to target players and mobs that you are standing on top of. In order to make sure that the development team has sufficient information to try to replicate this, though, can you please state the names of the specific mobs that you have experienced this with? Also, can you please state which areas have you experience this matter with? I understand that you stated this happened everywhere that you went, but in order to thoroughly investigate this matter, the development team would need areas and mobs to try to recreate what you have experienced. Otherwise, they may go to an area or investigate this situation with mobs that are not influenced with this matter, preventing them from being able to properly conduct an investigation. In addition, any information that you could provide in the following form would be a great help:

You can reproduce it even faster if you just run into a bigger mob (aldgoat for example), stand under them and then switch to active mode. AA will either tell you you are our of range or that you are too far away from the enemy.
One of the reasons I was against disabling collision detection...
