Well FFXI had 1 boss that only about 20linkshell globally could kill (PW), and 1 I boss I think 2-3 groups tops killed before the level increase (AV) both these bosses where out for 2+ years prior to the abysea game nerf. With CoP people would fail the normal storyline missions upwards of 10-20times before finally getting the strategy right to beat it, still to this date there is nothing in a mmo that has ever been released as hard as original CoP was that was not endgame. That and bosses that required 20-30 people to take down, and many fights that took upwards of 1-2hrs to finally win.
In the end almost no PvE is hard. All NPC controlled mobs and characters have a set way of acting, so once a solid strategy is down by the whole group it is a joke to kill. Fight something the first time it is hard as crap to kill, the 10th time it is a joke, the 20th time you can low man it, the 30-40th time you could probally duo/trio it if you where geared enough.
PW (the infamous 15+hr HNM of ffxi which really only takes about 100mins if you do not completely suck) is a prime example there where a top notch LS out that could kill it in about 80-100mins with 20 ppl with minimal deaths, and other linkshells that brought litterally 80-100 ppl to it would continuously wipe time and time again and ended up timing out at the 2hr point. Watching a video of a fight in FFXI makes it look like a complete joke, but then again most people who post video are not in the average linkshells. Even something as horribly boring as a tiamat/nidhogg fight where it is the same crap over and over is not easy at all the first time a group of people attempt it.
All I know in WoW I could go into a dungeon for the first time every and succeed without much trouble, hardmode not so much that took more focus, but anything hardcore in FFXI's endgame 1 person screwing up out of the group of 30 would possibly cause a 1-4hr ordeal to turn into a complete fail.
