I would double check to make sure everything is staying cool while the computer is on and/or in-game.
Check for unrestricted airflow on all of the fans and coolers, especially the ones on the video card.
Clean/blow out any dust build up or dust bunnies.
If the computer has a non-mesh-type inclosed case/tower, try running it with the side covers removed and see if the issue persists.

Also, check the disk health of your D drive.
CrystalDiskInfo is a great freeware program to perform that check.
