As a trans-woman (post-everything, married, successful, happy), it is a mental disorder. The stigma of mental disorder is a completely separate issue, but GID /GD is still a mental disorder.

The tricky thing is that the mental disorder is fixed by changing the unaffected body instead of the mental disorder. This is (I'm sure you know) because the gender in true-blue trans people is concrete. It's set, it is what it is. You cannot alter it, so after a long history of failures in trying to alter the mind, science has found that altering the body vastly improves quality of life, down to basic survival. The trans person, experiencing crippling GID / GD, the dysphoria will improve with cross-sex hormones.

That being said, the whole trans-person thing is still difficult physically, and the mental baggage of a life paused before transition is still bad, but it gets better after the mental disorder is appropriately treated.
