a) you route everything through their servers, adding more lag and strain to them
b) route directly between clients and have to deal with NAT issues ...
I can't speak for the PC version but the PS4 version of ESO only got text chat rather recently, like within the last year or so. Before that the only option you had to communicate was voice chat so if you planned to interact with anyone (even to just explain mechanics or ask why you had to tank an entire dungeon as a healer with an ice destro staff) voice was literally the only way to do so.
