how to gear depends on your resources.
if you're really good at making gil, you can buy i380 everything from the marketboard.
if you have a lot of playtime you can repeat content and "farm" for specific gear.
if you're not rich
- save two weeks of Genesis tomes and buy the body piece for your main role
- do Alphascape normal and roll only Springs in v3 and v4, after two weeks you'll have a pair of i380 pants.
- v1 and v2 you can kind of free roll but don't get more than 1 chain and 2 pedals in your first two weeks.
- do Suzaku Extreme 10x to get a weapon. (use Mendacity tomes, Sigmascape v4 Normal to get a 360 weapon if you don't already have one before this)
- week 3 Genesis tomes can be spent on Head/Hands/Feet, preferably the one with the best substats and whichever piece you don't have an Alphascape piece for.
- how you spend your Genesis tomes in Week 3 determines what you should roll on in Alphascape Normal week 3+.
You won't be high enough ilvl to queue for many of the things listed above once you hit 70 - first you'll have to work on that. As mentioned, you can buy crafted pieces off the marketboard if you have the gil. On my server, can generally say a piece will cost a million gil. The most recent crafted sets are i380. If you're not able to afford much but can buy something, a crafted weapon might be a good investment as getting weapons is a bit more annoying at first than getting armor.
Aside from crafted sets, you'll probably gear up in a combination of the following methods. First let's talk tomestones.
At level cap in XIV there are essentially two tomestone currencies in use. The current one, which is capped at 450/week, is Genesis Tomestones. Then there's the previous patch's tomestone, Mendacity, which have no weekly cap. (There's also Poetics Tomestones used for a variety of things, but not usually for gearing up at max level).
Nearly everything you do will give you Mendacity in some way shape or form if queued for at Max Level. You can use these tomestones to buy the previous patch's tomestone gear in Rhalgr's Reach, which is i360 by default and can be upgraded to i370. Some people join Hunt linkshells to kill Hunt marks to farm uncapped tomestones, but there are lots of ways (I've never entered Eureka/Pagos so if that's an option someone feel free to mention it - I don't even know if it has an ilvl requirement).
If you want, you can fully gear yourself up to queue for a lot of things via mendacity tomestones.
Genesis tomestones reward i390 gear that is upgradable to i400 (currently only through savage raiding), but it's limited per week.
As you accumulate both Genesis and Mendacity tomestones, use them to start gearing yourself up. Weapons are highest priority but need certain tokens obtained from the 8 man raids, Chests and Legs are next in terms of oomph per stat. However, accessories are the cheapest and bump your ilvl up, so if you want to brute force your way past some old ilvl requirements that can be an option.

Dungeons and Raids -
We're currently in the third raid tier of this expansion. I suggest unlocking the raids as you can, as you'll have to do the previous two 8-man raid tiers to unlock the current tier. There are also 24-man raids and I highly suggest finding and doing them. They are meant to be casual friendly content. Some story and dungeons are locked behind dungeons, so might as well do them and pick up any upgrades along the way, but they're not gonna be any long term pieces. In particular, once you unlock the second 24-man raid, the gear there is i360, equal to the mendacity gear. Via these raids are also tokens that will allow you to ultimately upgrade mendacity gear to 370, so once you're able to do the second 24-man, it's a good option to continue bringing yourself up to par.
The previous 8-man raid tier (on normal) will drop tokens that can be used for i350 gear. Feel free to supplement your gearing process with these, but again, first you'll have to unlock this tier and have the minimum ilvl to queue for it. Of note, the final boss of this tier drops a token that, along with 1000 mendacity, can be used to purchase a i360 weapon. I don't remember how many tokens it is, 4 or 7, but it's an option for weapons.
The current 8-man raid tier (on normal) will drop tokens that be used for i380 gear. Much of the playerbase is currently here - replacing 370 gear with new 380 pieces, and you could be here quickly via mendacity. Same thing with the final boss dropping a token that can be used to get a weapon, only it needs 1000 GENESIS tomestones and 7 kills (only one token per week for this) - so a lot of players are just waiting on that 7th week for their weapon right now.
But let's talk weapons, you've got a couple of options there.

Weapons.
Aside from progressive dungeon drops that are pretty meh, you can quickly buy a crafted weapon, farm previous raid tiers for the 1000 mendacity weapon (i360 and upgradeable to i370), or look to do an extreme primal. Ex Primals are good training grounds for Savage Raiding in FFXIV. However, since weapons are so important, it's gonna be rough to get into an extreme primal party without buffing the rest of your gear up and upgrading your weapon in some way, shape or form. The current Extreme drops i385 weapons and a token (10 of them buys a weapon of your choice so it's not RNG forever).
I'm not knowledgeable on the current Relic weapon via Eureka/Pagos. That might be an option but with all the horror stories, I don't know how feasible it is starting now compared to your other options. Someone familiar with the process may be able to elaborate!
Another option is to unlock Heaven On High, which you might have already been doing. You have a different weapon/armor set in there, and it levels up as you open chests/clear boss floors. You can continuously level them both down by 10 to create an item, and 10 of these items can be used to get an i365 weapon of your choice. This is convenient because your character's ilvl has NO bearing whatsoever in there. You'll start out weak, but before long your weapon/armor in there will be level 30-60 and you can keep doing it, leveling back up to something like 60 and dropping the gear down to 50 for a token. You may want to look into this, as it's a way to get a weapon without needing to increase your ilvl at all. This combined with mendacity tomestones (and the latest 24-man) is a surefire way to get your ilvl up to the 360s, which will easily let you join most max-level content at the time that opens the gateway to getting good enough gear to start savage raiding. Heaven on High also rewards tomestones (30 Genesis 50 Mendacity per run), so it can be a win/win if you feel like doing it for several hours. It's also something that can be done with any party composition and people use it for leveling, so you won't necessarily have to wait long for queues as a DPS.

Thanks to that, I got my i365 weapon, and in about three days worth of just doing it for 1-2 hours a day I got enough genesis to get my 390 chest piece
Then searching the markets... I found some 380 necklaces for 400k... kind of a steal since everything else was over 1 mil so bought that just for a quick boost. One of the dungeons I ran actually dropped 350 gear, so nothing to right home about, but it helped bring my ilevel up enough to do the new raid and Omega so I could work on better gear

How endgame raiding and gearing works in general is another topic, this post was mostly to give an idea of some ways to get your ilvl up to speed so you can participate in the newest content to begin with, but it's true that current endgame gearing involves the current Extreme for a 385 weapon, farming the current 8-man for 380 gear and tokens for an eventual 390 weapon, and either 370 gear (max from last tier) or crafted 380s. If you're really looking at progression raiding, that involves crafted sets of 380 with pentamelded materia, which is a hefty amount of gil to do for a bunch of pieces.

Right, so first things first, and that should be completing all your job quests. Not only does this provide you with new job gear (which can be upgraded via Eureka once unlocked, and if you can bear the tremendously tedious grind), it will also ensure all your abilities are available. Having these abilities is absolutely crucial in the endgame. Next, I also recommend completing the MSQ as well as unlocking non-story lv70 dungeons (such as Kugane Castle, Temple of the Fist, Fractal Continuum Hard, etc.). From this point, your main focus will be your gear's ilvl. Collecting gear from dungeons is helpful, but the main reason you'll want to unlock and run dungeons is for the tomestones they give for each clear, which can be exchanged for nice gear.
There are things you should know about tomestones: There are three types to collect. First there are Poetics, which have no bearing on the current endgame. Then you have the tomes for the previous tome gear, which are currently Tomestones of Mendacity, and finally the tomes for the current tome gear, Tomestones of Genesis. Lv70 dungeons will drop Mendacity and Genesis tomestones, as well as other endgame trials and raids. Note that while there's no limit to how much Mendacity you can collect in a week, Genesis tomes will be strictly capped at 450 tomes per week. Also, you can only carry a max of 2000 tomes each at any time. Once you've unlocked all the lv70 dungeons you will unlock the Level 70 Roulette and Expert Roulette, which will give bonus amounts of tomes on top of what the dungeons usually give out once a day. This along with running the most recent dungeons will be your primary means of grinding tomes, especially since running expert will net you 90 Genesis tomes a day. I'd suggest focusing on using your Mendacity tomes to speed you along the catch-up process, as you'll likely be getting loads of them thanks to first-time dungeon bonuses. You'll have to be a bit more savvy with how to decide using your Genesis tomes, given their limited availability, but each piece you get from them will be at i390.
As you start building up Mendacity gear, I recommend unlocking the Ivalice 24-man raids in Kugane as well. Each time you clear one of the raids, you will get a coin. You can exchange a coin from each raid at Rhalgr's Reach for items that can be used to upgrade your Mendacity gear. Your weapon will require a bit of work, however, and this will also tie in to the actual raid content. You'll want to first go through the Omega storyline, which can be started at Rhalgr's. Clear all the normal mode raids, and you'll have access to all the savage raids as well for each tier. To start, you'll probably want to farm up 1000 Mendacity and clear Sigmascape v4 normal four times to farm Sigmascape Crystalloids. This will allow you to exchange for the items needed to snag the i360 tome weapon, which can be upgraded with it's respective item that can be exchanged for Ivalice coins to the i370 weapon. Also, you'll want to consider running Alphascape normal for the parts drops, which can be exchanged for i380 gear at Rhalgr's. You can only win one part a week from Alphascape normal per fight, so roll wisely. Clearing the final fight will yield one crystalloid a week, and once you have seven you can exchange them and 1000 Genesis tomestones for the items needed to grab the i390 weapon.
I also recommend trying out the Ex trials first before delving too far into Savage content. Lakshmi, Susano, and Byakko are all good Ex fights to start getting a feel for the harder content without being too terribly unforgiving. Once you're up for it, unlock Suzaku Ex and try to take her down. Her weapons are all i385, which is great to have going into Savage. Also, each time you clear an Ex fight gives you one totem, and there's no lockouts. With ten totems, you can make an exchange for the weapon at, you guessed it, Rhalgr's Reach. It sounds like a lot at first, but trust me when I say that it'll probably take you very little time to catch up if you keep your due diligence, and you'll soon be ready to take on Alphascape Savage with the best of them. I also recommend taking some time to look up guides for your job to help you get a feel for how your rotation should flow, as well as watch some job PoVs of Ex fights and savage content. Learning, practicing, and improving will be invaluable to your progress. Good luck, and enjoy the endgame!
Further edit: There are also other ways to collect tomes besides dungeons, roulettes, and the like. If you want to shake things up, you can also get some decent amounts of tomes through hunts, the Uznair map dungeons, and even Eureka (again, if you can bear it).

You can buy i330 gear with Centurio Seals, as well as the items to upgrade them to i340. Because you can't get two of the same ring, you can only reach i338 that way, but any 350+ drop (or 340+ ring) from any content will get you to 340, which opens up everything except the most recent Omega raids and Four Lords fight, and savage. You can get enough seals (about 1,000) by doing your weeklies and all your dailies in both Heavensward and Stormblood for a few days (four at most, three if you get a weekly reset in the middle). Aside from buying gear, this is the easiest and fastest way to unlock Expert and all the other basics that let you get the actually good gear.
also if there're any cheap i350 crafted pieces left on the market you can buy some (regardless of which class they're for), exchange em for tokens at J'tandhaa in Rhalgr's Reach and then use those to get 360 gear for your class

Also assuming you're playing dragoon, a very robust part of your kit is locked behind the level 70 class quest so expect an adjustment period after you get it. Namely, you'll have to be more mindful of when you're using your jump skills and mirage dives (as well as keeping track of your botd timer.) Always feel like it's unfortunate that dragoons don't get to play with Nastrond before 70.
