Controller dying is a pretty lame excuse, but I think it's true sometimes. I cannot speak for everyone else's honesty, but I will give you one of my "excuse" stories.
One time my cat DID pee on me during a fight. That was definitely not a lie and I am sorry to the wipe I caused. It sounds far fetched and a bad excuse for dying... but she really did pee on me. I don't think anyone believed me. Or maybe they did because it was probably the first time they heard that one.
With that anecdote out of the way, it's really hard to tell when someone is lying about that kind of stuff. Many times I bet it's true and you should give people the benefit of the doubt.

Seeing as I've had a controller die on me before it actually can be a genuine excuse (though it only ever happened once). Was an old model PS3 controller (used on PC) and I believe that didn't have as good of battery capacity as the newer models. Though it was right as Turn 11 started so it was as trivial to fix as "plugging it in".
I won't tell you to believe them all but don't call everyone a liar and get mad if they snap back over the accusations.
My controller has actually died on a few occasions. So I will give the benefit of the doubt to them.

I think intent matters more than words do. If someone is embarrassed and they use an excuse to soften the blow, I think it is of little consequence. But since you asked the question, it may be that some people "lie" because (a) some people are rude when they make a mistake and (b) some people have an unforgiving, judgmental disposition and (c) some people are downright cyber bullies. Maybe they just don't want to deal with all of that. This was a question that didn't need to be asked.
Considering my FC mates have let their controllers die in the middle of raids while we're on comms, no, I don't believe everybody is lying on that. I just don't understand why they don't keep the charge cable nearby, or even notice the giant pop-up telling you "Controller on low battery" when on PS4.

I know it can happen that a controller dies and screws a player over, the same with batteries dying in wireless keyboards. I also know some people can be dishonest or unable to take responsibility for their actions (or lack thereof). So it's very likely some in Column A and some in Column B, with a severe lack of numbers or sample sizes to really show which is greater than the other.
Nah, not really. Batteries die at the worst moments often enough that I jumped off the wireless input bandwagon a year after it started and have stayed full wired since. And not just that, all the other general types of failures that can happen. I have a weird hardware conflict I've never found a solution for where my motherboard will for split-seconds stop recognizing mice, and only mice, making me lose control of whatever I was doing in-game for several seconds until it re-detects.

Depends. Is it an otherwise decent player who messed up one time, or is it just another excuse in a long line of them?
