This sounds like either a corrupted file or a connection issue. In that case, I'd suggest reinstalling the game. Make sure to delete the files in the 'My Documents' folder (My Documents > My Games > Final Fantasy XIV) when reinstalling as well as these may be excluded from a standard reinstallation.

If it's a connection issue, make sure your anti-virus/firewall has the proper exceptions for Final Fantasy XIV. Also make sure that the following ports are open:

54992 through 54994,
55006 through 55007,
55021 through 55040

Your ISP may forward these ports for you.

If these do not resolve the issue, please reply back a system report. Simply go to the 'Config' option on the Final Fantasy XIV launcher and to the right of 'System Information' click on 'Display.'
