I'm getting the exact same one. My earlier fix didn't stick. I've tried everything at this point. Reinstalling video drivers, reinstalling the game, removing all overlay sources, switching to dx9, setting folder to writeable, clearing system settings / documents folder. I'm convinced this is a client error. Given the nature of the crashes, I'm assuming its a character model or a piece of gear doing the damage, as it occurs only when I teleport to a new location, then I'm stuck out for a while, presumably until the offending person leaves the area. That's the only thing I can think of.

Actually getting two errors on startup now. The above one, and a second in its own window.

I was able to finally fix this - I deleted the game (again) deleted FFXIV folder that was in Documents as well and reinstalled on hard drive that has my OS installed. Now everything is fine. Hope this may help.

yup... been happening to me too off and on over the years, and since the last patch it has been happening regularly for no discernible reason. i can get up to go to the bathroom and come back to a crashed game with DX errors on screen. im not even going to bother going through the whole hoopla of uninstalling and reinstalling, it never fixes anything.

Was also able to finally correct this issue, I think. A full CC clean of the game's files and re-installation from there did it. Looks like the final cause was some corrupted index files.
