When did this start happening? Did anything change or update on your PC that coincides with this?

When you reinstalled the game, did you delete the files in the My Documents folder (My Documents > My Games > Final Fantasy XIV)? If not, try deleting this folder then relaunching the game.

Does the same error come up if you use DirectX 9 mode?

What else have you tried to alleviate the error?
