Cuchulainn. All of his variations in the history of Final Fantasy are creepy to me.
Someone just informed me of this:
They're voidsent (snurbles and kin), so they should creep you out! Only the fur actually belongs to the voidsent, whereas the eyes are those of its parasitized host, gazing in abject apathy at the fate of its life, being sucked away.
That's freaking terrifying...
I thought they were just a FFXIV monster. I don't remember seeing it on other games. It is the stuff of nightmares though....a fat, tentacled monster that vomits toxic goo on you.

For reference
Edit: Cuchulainn's variations are all different.
It appears as a Lucavi in FFT, and as an Esper in FFXII, both Ivalice games. Those are the ones I know of.
The Hecteyes are the grossest looking beasts in this game. The Giant Severed Heads in Yanxia are creepy as Hell as well.
The zombie goobue with the gaping maw for a stomach that eats you as you try to fight it.
As soon as I read the title of this thread, Toads popped into my head. Lo and behold, the very first post uses them as an example!
Tam Tara (Hard) boss is another good one, being a voidsent-animated severed head with little vein-arms.
I also think the Spectres in the Lochs are pretty creepy.
I forgot about this guy too...
It's really the sound it makes that creeps me out though. It's description is kind of sad too:
Flame Sergeant Dalvag was among the thousands of defenders who met the Garlean invasion force in northern Thanalan soon after the Calamity. His company was ill placed to hold their position, yet given strict orders not to retreat. Many fell, and suffered much in the falling. Multiple witnesses attest that Dalvag was the last of the company and fought as a man possessed till the end, the names of the dead tumbling in hoarse barks through cracked lips. Scholars theorize that his mortal remains are animated by a thirst for vengeance alone.
I don't know what would be worse...Getting bitten by that or getting beaten to death by it's earlobes...(it attacks you with them in-game)

^This. This still creeps me out utterly to this very day.
The plant-parasitized bears in Saint Moicaine's Arborteum run a very close second - a The Last of Us parody perhaps? Still utterly horrifying to look at.
