I know it's been a while since I personally touched on this subject, but I'm not sure it's so much that the devs are out of touch, it's that either them or Square itself that is afraid of major innovation. The most recent example I could think of was Eureka - surely, there was somebody on the team that had different ideas, but it felt like their fear of Eureka led them to ...create another Eureka with relics and nothing for DoH/DoLs. If it as truly as Yoshi P says and some of the requested features from the playerbase would create strain on the servers, my response to that would be, what was the whole point of moving the servers if not to lessen the strain on them? It feels like they are in a holding pattern, trying to reach 5.0 - it says as much when the last time Yoshi said something along the lines of 'if FFXIV survives past 5.0'. I dunno, that's not exactly a comforting thing to tell the playerbase that you want to retain.
