Generally speaking, the higher skilled people tend to be on top of things.
While not a hard rule by any means, it's just something I've noticed. As weeks go on your chances of doing something smoothly for the first time may drop.

Not always . I was on a alt on a learning party for Lakshmi extreme on pf. I notice that our main tank was a bad when he says to use discord on that easy fight and didn't tell any mechanics to anyone, also the noob was a little sarcastic. So I was there and told the mechanic for the other tank, for healers and dps. Even with that we wiped one time because people need a time to get it and it's normal. Of course that horrible tank left after one wipe. He was just there to be the first wolf thief. Glad that noob who thinks that are a ''god player'' is just another quitter. After we cleared we did more 7 times to farm wolf. Also that tank should know that on all MMO is a tanker job to explain the fight, if the tank isn't the new one of course.

I all most rage quit today from a healer not healing the tank and letting them die. Really gets to you fast when you have a healer and your rdm has to be main healer. Because healer only focus is dps.
If I'm RDM, I'll throw Vercures, sure. However sometimes I'll remind the healer that my DPS spells do way more damage than their rocks, and that they really should be doing their job so I can do mine. Or, you know, we can take 2x as long to kill crap because I'm forced to do their job. If that's what they REALLY want, then sure, I'll Vercure the whole stinking dungeon. I've done it before.
I'll only use heal/raise as an RDM under certain circumstances. If a healer is down I will raise them, if the tank is obviously about to die and healer is in trouble or down I will vercure. will sometimes heal myself if I was the one who stepped in it
If the actual healer wants to make an idiot of themselves, that's their lookout.
If he let the Tank die then he was an idiot, but if he is letting the Tank get low when he is DPSing that is perfectly normal for any healer that isn't garbage. I would also love it if Paladins would do their own rotation and not bother using Clemency when they fall under 40% hp. Some people need to trust their healers more and stop panicing.
People who seem like they are about to rage quit or express that "I'm just not good enough" attitude, I ALWAYS go out of my way to help. I try to have them tell me what they need what they don't understand I try to help them as best as I can so not only we can pass the fight, but to boost their confidence. Sometimes, I find that people who may not "preform well" MIGHT stem from them not being able to get over that giant wall of "I can't" "I'm just not good enough" etc etc. Now, people who reject help and just don't care at all...those are the ones that we should look at...maybe getting a character prepped to be a bot or something who knows -_-

At this point i'm convinced it's better to just heal myself when i get low than wait on a heal that doesn't come until it's too late.
Trust at this point is running very low.
Trusting the healer kills me way, way too often.
There seems to be some sort of belief that tanks can keep going indefinitely without healing and that any tank who dies as a result of being ignored by the healer is bad.

No, it's not. It's just some weird fallacy that doesn't even have basics in reality in my experience, as I never had a tank RL, even though I play MMOs for 15 years.
Tank is usually the one who knows dungeon tactics the best because players who don't know them are simply too scared to go as a tank.
That's weird -- my experience as a healer seems to be the opposite. Usually the tank pulls 3 groups at once then doesn't use any of their cooldowns so I'm stuck with no option but to Largesse+Lightspeed Benefic II or Largesse+PoM Cure II spam while their health violently ping pongs and I don't get to do any DPS at all. It seems to me it would be much more efficient for the tank to use a cooldown on big pulls so I can at least place some DoTs and get off a Holy or Gravity or two, seeing as those cooldowns are almost never needed for boss fights anyways, but what do I know?Most of the time i try this i end up dead.
