Correct, the game itself is rated with these terms stated (for the point of argument the sexual themes). Online can't be rated due to people (aka modders) able to change terms to a M rating right off the bat, what's my point? well...

If you pick up a game (for yourself or a minor) with a T rating that clearly stat that there is sexual themes in it's content, is it the games fault? No.
If you play an online game where modders can change the rating to a M or AO rating although they themselfs can't rate it, is it the games fault? again, No.

SE can only do so much and frankly as already stated, unless this minor is going out their way to get the mod themselfs to see these...pixels there isn't any harm in it. If your getting offended by these incidents well...again i bring you to the rating, look carefully at what you're buying and take into account that in this day and age mods are a thing.
