I have been stalked, and I've even been blackmailed (and that was in real life and had nothing to do with internet or games).
The real life incident was the most annoying one, but what did I do? I swallowed my pride for that day and went to the police. Result > they dealed with it for me.
Online stalking? boy that was easy just put them on blacklist, they make a secret character? I put them on blacklist again. Contact GM. It stopped soon enough.

As extra advice,
If somebody spams you with ERP talk, add to blacklist. You'll no longer see their messages.
If somebody harasses you take screenshots (very important) and you may want to NOT blacklist them immediately (so you have more screenshots of their talk and proof). Create a ticket and send in your proof, after that you can blacklist them.
From what I've heard, SE makes quite a big deal out of harassment, more then most others, and they will deal with it but you need to give them proof.
In my opinion, modding is fine. Actually, I like the idea of people tweaking some game textures to take more "control" over the personnalisation of their character.
Where I'm not fine, it's when it come to harassment. We all have different sensibility towards it, some will just shrug it off while other will take offense. I think it really depend on how deeply you immerse yourself in the game. And on that point, I can understand why OP is worried about it.
To me, modding is a chance as a PC player, and it's great that SE tolerate it. But if people start to complain, they will take action one way or an other and it will come as a real disappointment for a lot of people.
Be responsible and only take NSFW pictures of your own character and other who are fine with it, if you want to post it on a social media. We have nothing to gain, as a community, by defending this kind of behavior.

I would block them on all messaging app and stuff like Facebook for example. I would contact my phone service provider and ask them to block certain number for harassment (of course they can still call privately but anyway) and also to the police.
I had to do that for the blackmailing thing, it's not 100% the same as stalking but you can stop both and should go to the police especially if it's going extreme.
The more you care about people using mods, the more power you're giving them.
The worst thing you can do to a harasser is to give him attention. That encourages them and gives them some meaning.
Just ignore it, report if it happens ingame, blacklist elsewhere. The person they're taking screenshots from is not you, but a modded/fake version.
Also, I think devs can't solve this problem unless they invest a los of money to change the client, and that would take the money needed to make a new expansion, I believe.

Harassment that actually affects ones life is illegal in many countries. Under the guise of stalking, illegal threats or a select other laws. Someone harassed in real life over extended periods and with considerable severity (as in, not a random "You suck, idiot" here or there) should go to the police. They are the only ones that CAN do something about it.
Haven't been looking too deeply into this type of harassment...
But I do recall seeing mention of it online somewhere.

Someone had a link in their ffxiv description linking to an outside page of some sort with nude mods.
And something about harassing someone else with them...
IIRC, the GM's got involved, reprimanded them, and told them to remove the offending pictures or suffer further account action.
Not sure how accurate this is as I'm going off of memory from just coming across mention of it briefly.

Yes and she did, but regardless it took a long time for anything to happen and in the meantime she did contact GMs too because it was just as bad in game. GMs don’t do anything even about in game harassment with proof and not talking about “you suck” talking about real sexual harassment and not “I see you’re toon naked”.It's not that they won't do anything about real harassment. No one has given bad advice about how to handle harassment, just saying in this specific case of the OP I don’t see the GMs doing anything about it.
