How can I get promotion to Second Flame Leutenant?

Ive completed hunting log for GC
Completed all quests available
My squad is at lvl 59

Still says as bright as your flame burns no promotions available

What am I missing ???/

Unfortunately for now, First Flame Lieutenant is as high as you can get. Not sure when hey will increase it(possibly in 5.0)

Have you completed the Maelstrom quests? According to Google you need to complete the hunting log and a certain quest. You unlock the first one at Sergent First Class and it takes you to Dzamael Darkhold. And then the next quest takes you to Aurum Vale. Then you can apply for the promotion.

Like Avatre said before, First Lieutenant is the max rank until SE increases the ranks.
Second Lieutenant is one rank below First Lieutenant.

That's the rank before First Flame Lieutenant.

I just happened to check that rank list the other day, thinking "wow, that's a lot of ranks we can't get to."

Can't tell if it's a future implementation thing, or if some of those ranks are reserved for special NPCs or something.

I actually wouldn't be surprised to see us not go beyond High <insert GC> Commander. It's structured similar to the US military (and most military ranks honestly), and if that holds true, the Marshals would be equivalent to generals and admirals with the Champion beyond the Joint Chiefs of Staff. You ask me, the WoL has a bit too much on his plate to be handling entire national militaries. They might just be there for lore. But then, who knows, we may get there eventually.

That's kinda how I thought of it as well. Ranking too high, from a gameplay standpoint, wouldn't likely come with the responsibilities you would attribute with those ranks. Squads may be about as far as we can go into the leadership/management side of things.
