Monk Job Guide and Actions in-game

Some Monk Actions do not show the tooltip: "This action changes to XXX", but there still actions that we can't put in the hot bar.

I'm low level, and I do like to organize my hotbar previously, but I can't if I don't know which actions changes to what.

I will take Warrior for example: Inner Beast ※Action changes to Fell Cleave while under the effect of Deliverance.

Now for Monk, I expect that Shoulder Tackle will change to all the others elemental Tackle, but where are the "※Action changes to Earth Tackle while under the effect of Fist of Earth."?

And what about The Forbidden Chakra? It says ※This action cannot be assigned to a hotbar, but I can't figure out where is the primary action.

Meditation changes to Forbidden Chakra after you have 5 chakras, I believe.
