Party or solo: Party with only one friend since i entered in Eureka, so we shared the same EXP
Area and coordinates: Eureka
Steps:
1. I entered Eureka with a friend, since that moment we leveled together
2. We left Eureka after reaching lvl 2.
3. We entered again to Eureka and leveled up to lvl 3
4. We left Eureka again.
5. We entered again and leveled up to lvl 4
6. Then in the middle of a chain of 23, i got disconnected from the game with a server error.
7. I entered again, can't remember if in that moment i was still lvl 4, because i was checking if i lost the chain.
8. We left Eureka.
9. Today (16/03/2018) we went again to Eureka and i saw that i was lvl 2

Also lost levels but my materia too!
Logged in this afternoon, was lvl 4. Logged off for diner when I hit lvl 5. I had 3 pieces of materia which I got from the quest with Krile.
Logged back in after diner. Went into the Palace of the Dead for a few hours. Went back to Eureka and suddenly I was lvl 2! And the materia I got from Krile in the lvl 3 and 5 quests were gone!

I was halfway to lvl.7 and Server error Dc'd several times and finally got tired of it. When I came back in the morning I was level 5 again. Went and leveled up again and server errored again several times Went and left Eureka to do stuff in game for a while and came back later that night with friends and again I was level 5. Happened on the 13th-15th. Figured there was bugs, but I've reported other bugs to Enix and never got them resolved so why bother reporting, I'll just avoid the place until it's dead. Well now I've reported it in game and also here since there are several other people who also having problems. Hope Enix fixes their crap so I can actually keep up with my friends like an MMO is meant to be played.
