I'm using Windows 7, i7-920 (yep 1st generation), a GTX 970 and right now I'm running two copies of the game (1920x1200 borderless windowed with all settings on high and 1152x864 windowed with everything on standard quality) and NVIDIA System Monitor says I'm only using around 30% of my GPU.
Just really suggesting it might not be the game and might be a Windows 10 update that's the cause (because didn't that already happen once before with Game mode?)

Well, under best situations you always want maximum workload on your GPU. No 100% GPU usage would mean bad game That uses CPU for too much things That would normally do the GPU, lile unoptimized guild wars 2 or your CPU is just too weak for the GPU.
other games do this too, this is not an issue, this is normal IF you dont use any frame limiter. e.g. Assassin's Creed Origins, Battlefield or any other modern game.
but i agree in the term that something might be wrong with the ffxiv client, because most apps in Background immediately hang up with this game.

Win10 too and I have the same problem : GPU 99% => temps are high too.
I've uninstalled Geforce Experience, the high GPU usage is still here.
It might be the Windows 10 "Game Mode", it was on, I've set it off, but the problem is the same. I have no overlay.
I've downloaded old drivers from Nvidia yesterday I'll give them a try today.

Why you assume your temps are too high? Your GPU and CPU will auto slow down or increase fan speed to operate within a range.
If your PC overheat by playing FFXIV and shutdown (cause that would happen then), then your hardware is fault.

Can you also list the following:
The settings you are using for the game, just one of the presets will work (Maximum, Desktop: Standard, etc.)
Resolution
Max fps you get at 100% GPU usage (A rough estimate is fine)
How does this compare to the GPU utilization from before the patch?
