You mentioned that it only really happens in dungeons/trials/raids, does this occur when you first enter the instance, or while in the middle of playing? If you were to log back in immediately, do you have any errors? Where do you end up when you get logged back in?
What kind of connection do you have? Wired or wireless?
What have you tired so far to try an resolve the issue?

I also have been having random disconnections.
Once while trying to queue for a duty roulette.
Once more while waiting for the Gold Saucer GATE event to begin.

when i log back in (from stormblood game menu(not the launcher(it dont kick u that far out))) it just says logging you out because your conecting from a 2nd place

basicly everything (restarting the game, restarting the net, restarting the power, restarting the computer, ext)

If the game says "logging you out because you're connecting from a 2nd place," this suggests the disconnect comes from the client side as the server still thinks you are connected to the game.
Make sure that you do not have any other programs/overlays in the background and make an exception for any anti-virus or firewalls you may have.
Check the following Port Ranges on your router/modem to ensure that the following Port Ranges have been opened/forwarded/triggered:

54992 through 54994,
55006 through 55007,
55021 through 55040
*You may learn how to do this by visiting http://www.portforward.com/
Find your specific router's brand and model number from the 'List of Routers' and you will be provided a step-by-step guide on how to access these port settings.
Otherwise, your ISP may forward these ports for you.*

i dont hav any anti virus running / but i did hav 1 overlay running in passive i turned it off / im adding the ports just in case , if there is any more problems after this ill pm you again here.

ive done everything sept reinstal the game and im not doing that unless you wana buy be a sub.
