Please help, I've been trying to endure this for a month..

I have two issues, and I'm not sure if they're related or just intersecting issues.
1: About 50% of the time I freeze on load screens changing zones or zoning into a dungeon, it will freeze, FFXIV will stop responding and I will have to force close and relog. If left on this screen, it will stay loading indefinitely.
2: I have okay internet. I have -ZERO- issues with any other game, except for FFXIV. I get random disconnects, particularly in dungeons, I can sit in the middle of town for an entire day and never get disconnected but dare I try and do a dungeon, POTD is the worst by far, I'll get disconnected every minute and a half on a bad day.
As far as the first issue is concerned, I have reinstalled the game in an attempt to fix the freezing load screens, but it has not done anything for me. It shouldn't be a directx 11 problem as I have a computer designed for gaming and again no issues with any game outside of FFXIV.
It's been about 30 or so days of trying to muddle through this and see if I can figure out why on my own, but at this point I'm lost on these issues. I don't think port forwarding will fix the internet problem, and reinstalling didn't fix the freeze screen issue.

Can you elaborate on the issues you are having?

Please provide:

- Platform (PC, PS4, Mac)
- Any error codes or messages you receive upon disconnect (if any)
- Type of connection (wired or wireless)
- Other programs running while the game is playing (if any)
- Frequency at which disconnect or freezes occurs
- Where you are booted to after disconnect (Desktop, Start Screen, Character Select)
- Everything you have tried to resolve the issue

Can you also explain what happens when the game freezes? Is there any pop up message or does the game or device crash?
