Players, Non-Players, and Mobs not Appearing

I have been encountering this for a week now. I will be playing with someone and they will disappear from my screen or a certain mob will not load on my screen and I will aggro it without knowing and unable to target it unless I let it target me. This has happened to my brother who also plays. It's really annoying especially when you progress into a stronghold and you can't target a mob or ally let alone see them.
