A good start would be to remove all "instant death" effects with damage that's sufficient to guarantee a kill on an in-level player. Ultima from Ballad getting their fifth stack?! Instead of instant-death, have the attack deal serious damage. Lvl70 may survive it (or not), lvl50 won't. And yes, it is instant death, despite there being comments that it's supposedly 10 000 damage on next attack. I did "survive it" only for the screen to blacken out and the instance to restart anyway when I attempted it. Ultima at least can be soloed by a summoner, though it would take quite some time leaving all the killing to the summon. But Titan Hard cannot, seeing as its Gaol won't hit a summon, ever.

In the case of Gaol and similar imprisonment effects, just have it blow-up dealing massive damage if there are no players alive to destroy it. Again, the end result is identical for synced players (heck, I never, ever, saw anyone stay in a Gaol for more than a couple seconds anyway in Titan Hard), while it would make any class be able to solo it with a high enough level and base stats, as in, a level comparable to where other dungeons of similar grade are soloable.
