Hi Ya! Thank you for playing Final Fantasy XIV!

Based on the information provided, we have come to the conclusion that a complete reinstall of the game may be necessary. Please uninstall Final Fantasy XIV from your computer. Make sure to delete any remaining FFXIV files from your computer, including any potential files in your ‘Documents’ folder. Then reinstall the game from scratch. If you need a digital version of this, it can be found here: http://www.finalfantasyxiv.com/playersdownload/na/. Any updates or expansions will be downloaded to your computer once logged into the appropriate account.

Thank you for posting in the Final Fantasy XIV Technical Support Forums.
Please let us know if your issue gets resolved and what resolved it, So we can further assist other players.

Ariel said best as you are best to just uninstall and re-install.

However, if you still have the files (are they in windows.old)? Or did you reformat and move them? If you still have the FFXIV folder with all files contained, you can try making a shortcut to the ffxivboot.exe file and placing it on your desktop. This normally works if you didn't lose any of the files in the process, so if it doesn't, you will have to do a full delete / uninstall, and re-install, which I suggest you go ahead and do anyway to be on the safe side.
